Making a Claim


On your State Term Life Policy

To make a claim on a State Term Life (TLC) policy, please call a Sovereign claims specialist on 0800 500 195. The claims specialist will let you know exactly what information is needed to assess the claim.

 

For a Life Cover claim, some or all of the following may be required:

  • a full and complete death certificate,
  • a coroner’s report, if one was issued,
  • a completed discharge form,
  • a pre-printed deposit slip for the account that the benefit is to be paid into.

 

If the person who is insured was the sole owner of the policy, Sovereign will also require the grant of administration – probate or certificate of administration, or letters of administration. 

 

For a Living Cover or terminal illness claim, proof of the condition for which you are making the claim, such as medical evidence or reports or any other information may be required.

 

Important information:
Although most people can be covered, all applications are subject to individual consideration.  Special conditions, exclusions or premium loadings are sometimes applied.  This insurance is underwritten by Sovereign Assurance Company Limited.

 

 

On your Surgical Max Policy

 

Just give us a call on 0800 80 25 25 when you need to make a claim.

 

Contact us before you have the surgery, so we can approve your claim first – just call us on 0800 80 25 25 between 8am and 5pm, Monday to Friday.
We may require a medical report from your specialist, doctor or dentist before we can accept the claim.

 

The standard excess is $500.  If you have made several claims for treatment or procedures relating to a single health condition, you only pay the excess once.
